COVID-19 POLICY

In response to Alberta’s Relaunch Strategy which has lifted the Alberta Government’s mandate on the closure of certain businesses in Alberta, 2268168 Alberta Ltd. o/a Premier Cheer (hereinafter referred to as “Premier Cheer”) is planning to open for business.  In order to ensure the safety of our members, employees, contractors, staff members, guests and invitees, Premier Cheer has taken extra precautions and implemented new preventative measures which will assist in the prevention, exposure and spreading of COVID-19, such precautions and measures will include but are not limited to pre-screening of each member, employee, contractor, staff member, guest and invitee prior to entering the Premier Cheer facility, enhanced disinfection procedures and implementing personal protective equipment in accordance with the Government of Alberta’s Workplace Guidance for Business Owners and physical distancing when possible. 

ACKNOWLEDGMENT

I am aware of the symptoms of COVID-19, which include but are not limited to: fatique, cough, fever, shortness of breath, runny nose or sore throat. By my child’s voluntary membership and participation in Premier Cheer, I am acknowledging that:

1. I (or my child) has not and have not, nor have any of my household members, been experiencing any COVID-19 systems for the last fourteen (14) days. 

2. I (or my child) have not and no member of my household has been diagnosed with COVID-19 at any time.

3. I (or my child) have not and no member of my household has knowingly been exposed to anyone diagnosed with COVID-19.

4. I (or my child) have not and no member of my household has travelled to any areas that the Public Health Agency of Canada has issued travel advisories against within the last fourteen (14) days.

5. I (and my child) am aware of Alberta’s Chief Medical Officer of Health’s Order on March 25, 2020 (Order 05-2020) in regards to the requirement to self-quarantining when returning back into Canada from international travel and I (and my child) have and members of my household have complied with the self-quarantine requirements, if applicable.

6. Premier Cheer has encouraged me (and my child) to stay home if I (or my child) have any underlying medical conditions that could increase the severity of an exposure to COVID-19 and I on behalf of myself and my child acknowledge that I am not aware of any underlying medical condition that could increase the severity of an exposure to COVID-19 for myself or my child.

7. Premier Cheer has encouraged me to wear a personal protective face mask while attending any Premier Cheer activities, and if I (or my child) choose to wear or bring my own personal protective face mask, the personal protective face mask will be properly cleaned and sanitized prior to entering Premier Cheer’s facilities.

8. I understand that Premier Cheer is relying upon my honesty and Premier Cheer cannot be held liable for any exposure to the COVID-19 virus caused by any misinformation I have provided herein.

ASSUMPTION OF RISKS

I understand and I am aware that there are inherent risks and possible exposure dangers related to my child’s membership and participation in Premier Cheer, including my own attendance at a Premier Cheer activity, in relation to the contagious nature of COVID-19. By signing this Agreement I on behalf of myself and my child ASSUME ALL RISK that I or my child may be exposed to or infected by COVID-19 by my child’s membership and participation in Premier Cheer and that such exposure or infection may result in illness, personal injury, disability and/or death to myself or my child. I understand that my risk and my child’s risk of becoming exposed to or infected by COVID-19 while participating in Premier Cheer may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Premier Cheer’s employees, contractors, staff members, members, invitees or guests.  I on behalf of myself or my child voluntarily agree to ASSUME ALL RISKS, whether such risks are foreseen or unforeseen, general or specific, that arise from my child’s membership and participation in Premier Cheer or are increased by the nature of mine or my child’s pre-existing medical condition, and I accept sole responsibility for any illness or injury to myself or my child (including, but not limited to, personal injury, disability or death), damage, loss, claim, liability, or expense, of any kind, that I or my child may experience or incur in connection with my child’s membership and participation in Premier Cheer (collectively, the “Claims”).
